Are you passionate about making a real difference at a critical point in a patient’s care journey? We are seeking a dynamic and compassionate Occupational Therapists to join our Transfer of Care Hub, supporting timely and safe hospital discharges.
As part of a multidisciplinary team, you will play a pivotal role in assessing patients’ functional needs and facilitating smooth transitions from hospital to home. Working closely with health and social care professionals, you will ensure that discharge plans are person-centred, safe, and promote independence. We work with those citizens leaving hospital with a new or increased social need, as well as those with therapy needs.
Key Responsibilities:
* Conduct timely and holistic occupational therapy assessments for patients after discharge.
* Collaborate with your multidisciplinary colleagues to ensure safe transfer of care from hospital to community.
* Ensure safeguarding by identifying and addressing potential risks in the home environment, coordinating with relevant agencies, and implementing measures that protect vulnerable individuals and support a safe and sustainable return home.
* Contribute to reducing delayed discharges and improving patient flow.
* Identify and arrange appropriate equipment, adaptations, and support services.
* Promote reablement and use rehabilitative therapeutic approaches to reduce the level and duration of care packages through targeted interventions and collaborative goal-setting.
* Optimise care packages to ensure they are tailored, proportionate, and promote independence while supporting safe discharge.
* Working in partnership with our valued Lewisham Care Providers, including our Enablement Care Team.
* Promote wellbeing and independence, enabling individuals to live safely and confidently in their own homes or community setting
* Play a key role in our weekly Multi-disciplinary Team meetings a recommendations for future support and service
* Support the prevention of hospital readmissions by ensuring robust discharge planning, providing appropriate interventions and follow-up, and empowering patients with the skills, equipment, and support needed to manage safely at home.

What We’re Looking For
* HCPC-registered Occupational Therapist.
* Experience in hospital discharge, community care, or acute settings is desirable.
* Strong communication and problem-solving skills.
* Ability to work under pressure in a fast-paced, multidisciplinary environment.
* A commitment to person-centred care and promoting independence.
Location
The job is primarily based in the community with opportunity for hybrid working. Our main office base is at University Hospital Lewisham, with opportunities to work remotely in other locations within the borough including Laurence House.

Salary
£44,430 - £46, 437 per annum (SCP 31-33)
N.B: All appointments will be offered at the minimum spinal column point (SCP) for the grade of the role. The only exception to this will be when matching current salary (up to the maximum of the scale), which is subject to evidential proof.
