This is an exciting opportunity for an Occupational Therapist to join the Transforming Lives Team – Prisons. As a valued member of this team, you will need to have a good understanding and experience of the application of the Care Act 2014, Mental Capacity Act 2005. You will have experience of working creatively with adults who have care and support needs and will bring a commitment to the principles of strengths‑based practice, promoting independence and wellbeing whilst safely managing risks. You will work within a wider Social Care Team and Directorate, including Community Forensic Social Work teams and the Mental Health Discharge Team.

Clinical Occupational Therapy supervision will be provided by a Senior Occupational Therapist and you will have the opportunity to link in with the wider Occupational Therapy teams within Nottinghamshire County Council.

Key Responsibilities

* You will undertake occupational therapy assessments and interventions of adult prisoners within Nottinghamshire, providing care and support where those needs meet eligibility criteria (Care Act 2014), and transferring that care back into the community when their sentence ends.
* You may also be required to undertake assessments in hospitals and community settings, assessing for and prescribing equipment, carry out environmental assessments of property to identify suitable housing and carry out functional assessments to identify appropriate Occupational Therapy outcomes.
* Assess and review to identify ongoing needs and commission appropriate services, e.g. equipment, adaptations, techniques, care packages, goal setting in order to maximise independence, improve quality of life for people with support needs and informal carers, and reduce, prevent or delay the need for formal or residential care in line with the Care Act (2014).

As an Ideal Candidate, You Will Be

* A qualified Occupational Therapist, registered with the Health and Care Professionals Council.
* Experienced Occupational Therapist.
* Have effective communication, negotiation, organisational and IT skills.
* Be committed to interprofessional collaborations with partner agencies.
* Be able to work autonomously and have a high degree of resilience.
* Able to drive and holds UK Driving Licence (except where disability precludes this).
