We are recruiting two Band 6 Hospital at Home Practitioners based within the Harrow Integrated Care Team at Stanmore. This is an opportunity to join an innovative community service delivering acute, hospital-level care in patients' homes.

Central London Community Healthcare NHS Trust is rated Good by the Care Quality Commission and delivers high-quality, person-centred care across London and surrounding areas.

The Hospital at Home service supports adults with acute conditions through multidisciplinary care, including home-based monitoring, point-of-care testing, and intravenous therapy. Practitioners work autonomously while being fully supported by an experienced team.

We welcome applications from compassionate and motivated practitioners keen to develop their skills in a forward-thinking service.

The post holder will work autonomously and in collaboration with Acute Trusts, the Adult Community Nursing Team, other health care professionals and statutory and voluntary agencies to deliver excellent standards of patient care.

The post holder will be required to:

Work on a rotational shift basis within the Harrow – service operating hours 8 am – 8 pm, 7 days a week.

Prioritise and manage own workload with minimal supervision.

Case Management Clinicians and Community Therapy Multi-Disciplinary Team

Other Brent and Harrow CLCH Trust Specialist Teams (including Unplanned Care/ Urgent Community Response/ Discharge to Assess, Tissue Viability Nursing, Speech and Language Therapy, Dietitians, Diabetes Management, Bladder and Continence, MSK)

External (if applicable)

Central Remote Monitoring Hub – for ongoing virtual ward management and reviews

Primary Care Services

Secondary Care Services

Patients, Family members and Carers

GP's and Practice Nurses

Adult Social Services (Learning Difficulties, Care Agencies)

Social prescribers and Voluntary Sector Organisations (Age UK, Red Cross)

LAS and NHS 111

Trust and GP Out of Hour Service

Local Rehabilitation Units and acute hospitals

Care homes

Wheelchair services
