A Health Care Support Worker works alongside a team with the Care Home Team, 18 care homes and wider PCN teams.

We have a permanent vacancy for a Health Care Support Worker to join our large, supportive, busy and friendly team. The successful candidate will play an important role in complementing our current Care Home Team and other members of the PCN multi-disciplinary team.


Main duties of the job

We are seeking an experienced HealthCare Support Worker who has a passion for care of the elderly and experience ofworking in the community. They must be able to work autonomously in theirclinical work delivering reactive and proactive care to care home residents. Weare looking for an individual who will help support our established Care Hometeam, alongside ANPs & Care Co-Ordinators. Other members of the wider PCNteam will support you in this.

North Lincolnshire South PCN is a large, forward-thinkingPrimary Care Network compromising 6 practices across Scunthorpe and thesurrounding area and providing services for over 74,000 patients. Our aim is toprovide exemplary patient care; finding innovative solutions in generalpractice to deliver the best care we can to our patients.

The six practices comprise: Ancora Medical Practice, AshbyTurn Primary Care Centre, Cambridge Avenue Medical Centre, Cedar MedicalPractice, Kirton Lindsey and Scotter Surgeries, West Common Lane TeachingPractice.


Job responsibilities

North Lincolnshire South Primary CareNetwork (PCN) is looking to expand their Care Home Multidisciplinary Team. Weare recruiting an experienced Health Care Support Worker for our care home team in both the support ofpatient care and work in line with the new Network Contract Directed EnhancedService (DES) Specification The post holder will be delivering care anddeveloping the service according to the principles of the Enhanced Heath inCare Homes framework and working closely with practices to deliver outstandingpatient care.

The post holder will work closely with 18 Care Homes and the 6practices, Ancora Medical Practice, Ashby Turn Primary Care Centre, CedarMedical Practice, Cambridge Avenue Medical Centre, Kirton & Scotter Surgeryand West Common Lane within the PCN and other local organisations.

To Carry out clinical assessments on residents.

* Abilityto carry out a Blood pressure checks on a patient and be able to act on theresults.
* Abilityto carry out venepuncture or be willing to undertake training
* Abilityto follow protocols and work within scope of competency and feed back toregistered nurse
* Abilityto use own initiative and work without close supervision
* Ableto listen effectively and demonstrate a sympathetic/ sensitive approach. Ableto speak effectively to the wider MDT/ patients and families
* Workingwithin guidelines for, undertake and record the following tasks when reviewinga patient
* Oxygensaturations
* Pulserate and rhythm
* Temperature
* Heightand weight/MUAC
* Waistcircumference and BMI
* Limbmeasurements and assessment
* Bloodglucose monitoring
* Historydocumentation
* Peakflow readings
* Ableto use and complete clinical tools to aid registered nurse decision making
* Tosupport in the delivery of enhanced services and other service requirements onbehalf of the PCN
* Whenrequired to attend and participate in the delivery of multi-disciplinary teams(MDT) within PCNs.
* Toundertake all mandatory training and induction programmes
* Tocontribute to and embrace the spectrum of clinical governance
* Todevelop yourself and the role through participation in training and serviceredesign activities
* Toattend a formal appraisal with their manager at least every 12 months. Once aperformance/training objective has been set, progress will be reviewed on aregular basis so that new objectives can be agreed.
* Tocontribute to public health campaigns (e.g. flu/Covid clinics) through adviceor direct care
* Tomaintain a clean, tidy, effective working area at all times
* Supportdelivery of QOF, incentive schemes and other quality or cost effectivenessinitiatives
* Undertakeany tasks consistent with the level of the post and the scope of the role,ensuring that work is delivered in a timely and effective manner
* Dutiesmay vary from time to time without changing the general character of the postor the level of responsibility
* Workwithin a team & own initiative to work independently
* AttendWeekly debriefs and monthly team meetings on Wednesdays.


Person Specification


Qualifications

* Candidate must hold Education Level 3 NVQ with Evidence.
* Experience of taking clinical observations and interpreting the readings.
* Clean Driving License and own transport.
* Ability to follow protocols and work within scope of competency and feed back to ANP/ Clinical Director.
* Able to communicate in a sensitive and reassuring manner, adhering to Confidentiality at all times.
* Trustworthy and approachable manner
* Flexibility within working environment.
* Commitment and motivation
* To be prepared to develop further knowledge beyond the basic requirement of the position
* Ability to organise own diary and prioritise tasks
* Qualified in Phlebotomy or willing to learn.


Experience

* Candidate must hold Education Level 3 NVQ with Evidence.
* Previous experience of working as a healthcare assistant in either a hospital/registered care/community setting or GP practice.
