We’re launching an innovative community care centre inspired by world-class models like the Trieste Mental Health Community Network, Navigo in Grimsby, and Clubhouse models in New York and Brixton.
We have an exciting and rare opportunity to be part of a new community mental health pilot. This centre will have a major national profile with NHS England, who are hoping that the pilot will radically transform how mental health care is delivered in the UK.
We are looking for dedicated staff to help pioneer this revolutionary approach to care.

* Support the team with overall management of referrals, providing screening, triage, and trusted assessments for patients with a range of mental disorders.
* Work in collaboration with partner agencies and the multidisciplinary team (MDT) to provide interventions supporting service users to stay well within their communities. Use evidence-based tools to inform care planning and support engagement with treatment and services.
* Support family and carers with risk management and escalate concerns as needed, ensuring clear communication of risks involved in care.
* Collaborate with community services using shared decision-making to ensure safe and effective discharge planning or onward referral, including to primary care, social prescribers, third sector, voluntary services, or community groups. Ensure timely sharing of care plans with all involved parties.
* Participate in MDT governance, development, and clinical meetings.
Applicants should have significant knowledge and understanding of Mental Health Law, including relevant legislation such as the Community Care Act (2015) and treatment legislation. The role involves providing low-intensity CBT and Mindfulness interventions under supervision, as well as psychoeducation to service users.
