Job Title: Specialist Occupational Therapist – Reablement

Job Summary:
The Specialist Occupational Therapist will deliver a person-centred Reablement service aimed at supporting individuals to regain independence in their own homes following a health or social care crisis. Working across community settings, the post holder will assess, plan, and implement interventions to help clients perform essential daily living activities such as personal care, mobility, and meal preparation.
You will manage a caseload of clients with complex needs, develop tailored treatment plans, and prescribe equipment or minor adaptations as required. The role involves working collaboratively with clients, carers, support staff, and multidisciplinary teams to ensure effective, goal-directed rehabilitation. Communication skills and a strong client-centred approach are essential.
Key responsibilities include clinical caseload management, line management of junior/support staff, and active participation in service development and improvement. The post holder will also support the assessment and review of domiciliary care packages and contribute to quality assurance and audit processes.
You will participate in professional supervision, training, and continuous professional development, while also acting as a Practice Educator for students. Flexibility is required, including potential weekend or extended hours to meet service needs.
Essential Criteria:
* Degree/Diploma in Occupational Therapy
* Registered or eligible for HCPC registration
* Minimum 2 years post-qualification experience, including 6+ months working with older adults in acute/community settings
* Full UK driving licence (unless disability exempt)
Desirable Criteria:
* Postgraduate training in rehabilitation/falls prevention
* Practice educator experience
* Knowledge of cognitive/perceptual deficits
