The people we support may have spent an extended period in hospital or another setting, therefore we utilise a person-centred approach; empowering individuals to become as independent as possible within their own homes
About the Role
The 'DORS' service will facilitate the discharge of individuals from psychiatric hospital. It offers time-limited, specialist mental health support as individuals transfer from acute settings to virtual wards as a mechanism to improve the patient journey during transfer of care. This service can also support people who are currently in a community setting, such as their own home, and experiencing a period of crisis which has the potential to lead to admission.
Our team of Recovery Workers are based across West Sussex, working with individuals to support them wherever they call home. We recognize that when a person may be experiencing a time of difficulty it is important to offer flexibility, dedication and compassion; therefore, these are the traits that we look for in members of our team.
Job Opportunity
amazing opportunity to work with our specialist discharge team working in partnership with the NHS supporting clients on immediate discharge from hospital, or working with those in crisis to avoid re-admission.
The role in very fast paced as each client works on an intensive six week recovery programme with key goals to achieve, before either moving on from our support independently, or working with one of our community based teams to access longer term support. You will be covering the entirety of West Sussex so your own transport is essential for this role - an expenses policy is in operation to cover travel outside of your commute.
Key Duties
* To facilitate the delivery of a service which follows a person-centred ethos and promotes independence.
* To complete dynamic risk assessments and support plans.
* Promote wellbeing and appropriately manage risk.
* Ensure adherence to company and national policies and procedures.
* To work flexibly as required by the service; this may require some working outside of your normal hours to provide consistent support.

Essential Skills
Essential skills & experience:
* Experience of supporting a person(s) with a mental health condition.
* Experience of working within community-based services
* Ability to directly and indirectly support people in ways that promote equality and empowerment.
* Demonstrate the ability to meet deadlines and respond to changes in an individual's needs
* Excellent interpersonal skills and the ability to develop a rapport with service users and partners.
* Ability to positively address and challenge inappropriate or negative behaviours and attitudes.
* Establishing and coordinating positive links to the local community, activities and opportunities for the people we support
* Full clean driving license and own transport to work flexibly within community setting
Desirable:
Qualification in Psychology, Occupational Therapy, Nursing or other discipline
Level Three+ Diploma or equivalent in Health and Social Care
