Salary: £40,192 - £44,012 per annum, plus an annual retention bonus of £3,000 subject to eligibility. Job Type: Full Time. Location: Maidenhead.
The Role
As a Social Worker within the Hospital team, you will support adults to live independently and safely by assessing needs, coordinating care, and safeguarding wellbeing. You will work closely with individuals, families, and partner agencies to promote dignity, choice, and empowerment.
Your Responsibilities
* Support & Advocacy: Assist adults vulnerable due to age, disability, illness, or mental health challenges to live independently and with dignity.
* Assessment & Planning: Conduct strengths‑based assessments and develop person‑centred care plans.
* Safeguarding: Protect adults from abuse, neglect, or exploitation and collaborate with other agencies when risks are identified.
* Coordination: Liaise with families, carers, health professionals, and community services to ensure holistic support.
* Empowerment: Promote choice and control, helping people understand their rights and access services.
Qualifications & Experience
* Degree in Social Work and registration with Social Work England.
* Strong understanding of adult safeguarding procedures and legislation (e.g., Care Act 2014).
* Excellent communication and collaboration skills to work effectively with individuals, families, and multi‑agency teams.
* Capacity to manage complex cases, maintain professional boundaries, and adapt to changing demands.
