We're Hiring: Social Prescribing Link Worker
Location: Birmingham B27, B28 and B90

Salary:£26,000.00 - £28,000.00 DOE (FTE 40 HOURS)

About the Role
As a Social Prescribing Link Worker, you’ll empower individuals to take control of their health and wellbeing by connecting them to non-medical community resources. Working alongside GPs, Advanced Nurse Practitioners, and First Contact Physiotherapists, you’ll take a holistic approach to supporting people with complex social needs.
Key Responsibilities
* Manage a caseload of individuals referred from primary care
* Conduct comprehensive assessments and develop personalised care plans
* Identify barriers to wellbeing and connect people to relevant community services
* Use coaching techniques to promote self-management and behaviour change
* Build relationships with local groups and help develop new community initiatives
Skills & Experience
Essential:
* NVQ Level 3 in Social Prescribing (or willingness to work towards)
* Experience in health, social care, or community development
* Strong communication, coaching, and rapport-building skills
* Ability to work independently and collaboratively within a multidisciplinary team
* Understanding of social determinants of health and long-term condition management
Desirable:
* Training in health coaching, motivational interviewing, or behaviour change techniques
