As a Nurse Case Manager RGN, you’ll coordinate complex-care packages for adults and children, using your clinical expertise to assess, plan, and manage person-centred care in home and community settings. Key Responsibilities Manage a caseload of community complex-care clients across the Coventry region, ensuring safe and effective delivery. Carry out clinical assessments, risk reviews, and care-plan updates in line with best practice and CQC standards. Provide clinical supervision, mentorship, and competency sign-offs for carers and support staff. Conduct clinical audits, medication checks, and quality reviews to maintain compliance and governance. Liaise with families, ICBs, case managers, and MDT professionals to coordinate holistic care. Deliver training and clinical support to teams during both routine and emergency situations. Drive continuous improvement and uphold professional nursing standards as a Nurse Case Manager RGN.
