Job overview We have an exciting opportunity to join the Spalding Integrated Placed Based team which is part off Lincolnshire Partnership NHS Foundation Trust’s Adult Community services. The Placed Based Team work closely with the Community mental health team and Neighbourhood team as part of the transformation of mental health services across the local primary care network. The role facilitates delivery of wellbeing-focused psychologically-informed interventions, aligned to cognitive-behavioural principles, based on the best evidence available, that address problems often experienced by people with severe mental health problems. You will work under close case management supervision from a suitably qualified professional to provide high quality collaborative care planning as a member of the multi-disciplinary team. Also you will receive clinical skills supervision from an HCPC registered clinical psychologist or BABCP accredited cognitive behavioural therapist to provide specified psychologically-informed wellbeing-focused interventions. Advert To carry out holistic biopsychosocial assessments and facilitate delivery of wellbeing focused psychologically informed interventions based on the individuals assessed needs and following cognitive Behavioural principles. Coordinate care, assess risk and refer appropriately and co-ordinate the patient's journey across the health, social care and voluntary sector system.
