Job Summary

We are looking for a dynamic, experienced Advanced Nurse Practitioner. You will work collaboratively with our staff and the wider University Support Services in order to support students to achieve a successful academic outcome and develop skills for achieving and maintaining a healthy life.

Job Summary

We are looking for a dynamic, experienced Advanced Nurse Practitioner. You will be a team player, who is passionate about delivering high quality care contributing to the health & wellbeing of our (predominantly) student population and their families. You will work collaboratively with our staff and the wider University Support Services in order to support students to achieve a successful academic outcome and develop skills for achieving and maintaining a healthy life.

As an Advanced Nurse Practitioner you will be a registered healthcare professional with your digital badge. You will be used to practising autonomously using the skills of advanced clinical assessment, examination, diagnosis and treatment within an agreed scope of practice, which will include minor illness and patient triage. You will work with the Acting Nurse Managers, the Nursing team and the supportive Multidisciplinary team here at UHS to ensure the best care for our patients.

Main duties of the job

You will be passionate about the health and wellbeing of our student population, and will want to deliver high quality NHS services to our patient population.

We are looking for someone who wants to work as part of a team of advanced practitioners to provide a first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem solving and clinical decision-making, to establish a diagnosis and appropriate management plan. You will also undertake triage, both by telephone, online and face to face, for patients seeking access to services.

About Us

The practice population is made up of mainly students and their families, with 98% of practice falling in the age range 18-40. This provides a unique opportunity for a clinician with an interest in a wide range of conditions, particularly those relevant to our patient population, including mental health, sexual health and contraception choices.

You will work collaboratively with our staff in order to support students to achieve a successful academic outcome and develop skills for achieving and maintaining a healthy life.

We are located in a bright and airy purpose-built building situated on the University of Sheffield campus, which is also close to lots of local amenities and the buzz of City centre life.

Job responsibilities

* Provide a first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem solving and clinical decision-making, to establish a diagnosis and appropriate management plan.
* Practice within scope of professional registration (e.g. NMC).
* Undertake triage, (including online), for patients seeking access to services.
* Provide safe, evidence-based, cost-effective, individualised patient care, referring patients directly to other services/agencies as appropriate.
* Manage caseload and clinical priorities according to agreed protocols and working practices.
* Within scope of practice and clinical competence request and/or undertake diagnostic procedures and clinical investigations related to plans of care.
* Have a working knowledge of agreed national and local guidelines for clinical practice.
* Working as part of the wider practice team, utilise advanced knowledge and skills to provide advice and support to other members of the multidisciplinary team. Where clinically appropriate, implement and action changes within the patient medical record
* Liaise with doctors and other healthcare professionals in secondary care in order to provide the optimum care for your patient.
* Act as a Non-Medical prescriber, working within clinical competencies at all time, and to your Professional body, adhere to the Standards for Competency Framework and local and national guidance.
* Regularly attend clinical governance sessions and take part in audits of yours and others practice in order to maintain safe and effective practice.
* Use professional judgment to act as an advocate for patients and ensure a patient centred approach to the delivery of care. Support and enable patients and carers to make informed decisions relating to their treatment and management. Escalate any concerns via the nursing/professional structures are required.
* Ensure high standards of documentation. Records should be appropriate, complete and up-to-date and should adhere to the principles of the practice confidentiality and data protection policies.
* Occasional work out of hours to support vaccination campaigns, health promotion campaigns and other events within the University
* Work with the general practice teams to ensure the practice is compliant with relevant CQC standards.
* Work with your line manager to access regular clinical supervision, to enable you to deal effectively with the difficult issues that people present.
* Work collaboratively, striving for excellence within the nursing team wider UHS.

Essential

* - Understanding of importance of evidence-based practice and ability to follow clinical policy and procedures, including an understanding of the audit process and ability to evaluate the safety and effectiveness of own clinical practice.
* - Ability to effectively triage, assess and diagnose.
* - Excellent communication skills (written and oral) including recording of accurate clinical notes and be able to effectively communicate and understand the needs of the patient, listening and empathising whilst being honest and caring.

* Evidence of working autonomously and ability to assess and manage patient risk effectively and safely, working within own scope of practice, with some support and supervision.
