Overview
We are looking to recruit a motivated and enthusiastic Mental Health Practitioner to join the Mental Health Intensive Support Team (MhIST), an award winning and forward-thinking intensive community mental health rehabilitation service. The post will be based in Macclesfield. MhIST is a recovery-orientated service that offers specialist assessment, intervention and support to individuals with complex mental health needs across Cheshire & Wirral. The team works alongside patients, families and social networks with a focus on meaningful rehabilitation goals and supporting the individual in their recovery journey. The service provides opportunities for ongoing professional development, including quarterly team training days and protected reflective spaces. We welcome applicants from Nursing, Occupational Therapy and Social Work, and from other professional disciplines with relevant experience.
Responsibilities
* Work intensively with a small cohort of patients with complex mental health needs, delivering high-quality value‑oriented care.
* Demonstrate excellent skills in risk management, adaptive care planning and effective communication to support patients in remaining in the community.
* Collaborate with a multidisciplinary team to provide holistic, need‑adapted community rehabilitation.
* Engage in proactive approaches to crisis prevention and community-based support.
* Contribute to service development and professional learning within MhIST.

Qualifications and Experience
* Essential: Educated to diploma/degree level and registered with the appropriate professional body; relevant degree knowledge plus experience with complex cases; minimum of 2 years post-registration experience with adults with mental health problems of a complex nature.
* Desirable: Evidence of continuing professional development; additional qualifications (e.g., NVQ A1, SPQ, mentorship qualification) as required by governing bodies; experience of delivering psychosocial interventions and working within recovery-focused frameworks; knowledge of the Mental Health Act and Mental Capacity Act (Desirable).
* Experience of planning and organizing complex activities or programmes; experience of developing specialist care packages; experience working with commissioners and care providers; experience of supervision and audits as appropriate.
Knowledge and Skills
* In-depth knowledge of recovery-focused practice and delivering individual, service-user focused assessments.
* Ability to apply evidence-based interventions that promote recovery pathways and measurable outcomes.
* Knowledge of physical health needs and developing/monitoring care programmes.
* Ability to work with and alongside carers, families and social networks to support rehabilitation goals.
* Strong risk assessment, communication and documentation skills; ability to produce and adapt care plans accordingly.
