Job Description:
The Role
The main role of the Throughcare and Aftercare Nurse is to support the foundations of the Promise and young people in receipt of Throughcare and Aftercare. A primary function of this role is to provide help and support to assist care leavers in achieving the best possible outcomes and transitioning successfully into adulthood. Support will focus on improved assessment of health needs, review of support pathways, and promoting self-efficacy. The young people supported are up to 25 years old (until their 26th birthday). The postholder will work closely with the Throughcare and Aftercare team and all relevant partners and agencies.
The postholder will be responsible and accountable for maintaining all clinical, staff, and information governance standards.

Registration requirements
* Registered nurse at degree level or equivalent
Qualifications & Knowledge
* Educated to Masters level in a relevant nursing area, or
* Equivalent knowledge demonstrated through NES modules on Assessment, Leadership, and Critical Thinking.
* Knowledge of legislation related to information sharing, data storage, and public protection
* Understanding of health and social care pathways related to safeguarding, early intervention, and protection
Qualities and Skills
* Experience of caseload management and planning
* Experience working with families using a strength-based approach
* Ability to use assessment tools for referral decisions
* Risk management and critical thinking skills
* Ability to provide advice in multi-agency settings prioritizing children's safety
* Leadership in discussions and decision-making in complex situations
* Development and implementation of policies and service standards
* Commitment to ongoing professional development
* Must hold a full UK/EU/EEA driving licence if required
Preferred qualifications and experience
* SPQ in related field
* Experience with care-experienced children or young people
* Knowledge of quality improvement methodologies
* Understanding of The Promise and UNCRC
Contract details
Permanent, full-time, 37 hours per week
Location and Working Pattern
Based in School Nursing within Cambuslang Clinic. Candidates may be considered for similar roles in other locations.
