Are you organised, caring, and passionate about supporting patients and improving healthcare services? We're looking for a dedicated Admin & Care Coordinator to join our friendly team.
In this role, you'll provide essential administrative support while helping patients navigate their care. From coordinating appointments and managing medical records, to supporting patients with long-term conditions and promoting key health programmes, you'll play a vital part in ensuring our patients receive the best possible care across both of our sites.
What you'll do:
* Act as the first point of contact for medical records requests, ensuring confidentiality and compliance with GDPR.
* Support patients with health campaigns, medication reviews appts, and care plans.
* Coordinate care across practices, care homes, and health services.
* Maintain accurate medical records, coding, and screening data.
* Provide excellent administrative support, including emails, scanning, scheduling, and reporting.
* Work closely with GPs, nurses, and the admin team to meet practice and PCN goals.
* Be flexible and reliable while working across both sites.
