Due to recent expansion and investment, The Preparing for Adulthood (PfA) Team has a number of exciting opportunities to join the team.
The Preparing for Adulthood team has recently been reconfigured into three teams within one service, based in adults’ services but aligned with the children’s service to support partnership and seamless transition to adulthood. These include:
* Preparing for Adulthood: Living at Home
* Preparing for Adulthood: Leaving Care
* Preparing for Adulthood: Complex
We work directly with young people from age 16 (and in schools from age 14). Our focus is to support their transition and preparation for adulthood, working with those with Learning Disabilities, Difficulties, Autism, ADHD, physical disabilities, health needs, and complex trauma or attachment issues. No two days are the same. We are person-centred, aiming to prevent or delay long-term services, and when needed, work with young people and their support circles to ensure appropriate support to achieve their goals.
The team works across children and adult services, offering a challenging and exciting landscape, especially with opportunities to shape future services.
Manchester has invested in Person Centred Planners who will work alongside young people, carers, and social workers to develop tailored plans and directly support young people in achieving their goals, making a real difference in their lives.
We seek enthusiastic, compassionate individuals who want to see young people succeed and champion their rights, especially those with SEND. Creativity in supporting young people's achievement is encouraged.
We welcome those open to trying new approaches and engaging actively.
Initially, you will work across all teams to develop your skills in Preparing for Adulthood, legislative frameworks, and Manchester’s practices. This will help us match you to a team aligned with your passions and skills. We also offer team rotations to prevent burnout and maintain skill levels.
Our base is Etrop Court, Wythenshawe, but support is provided city-wide and beyond. The role involves 35 hours per week, with a hybrid working model of office and home, including visits. You will be instrumental in supporting young people's life planning as they transition to adulthood.

You must have:
* A professional qualification in social work
* SWE registration
* Enhanced DBS/Update service
* References for the last 5 years of employment
