Job Description
This is an exciting time to join The London Borough of Enfield (LBE) as we continue to work in partnership with our Health Colleagues to develop and strengthen our services to vulnerable Adults in accordance with the Care Act 2014 requirements. Enfield currently have Social Work position available in the Assessment and Prevention Hub within Older People and Physical Disabilities service. Our team consists of Social Workers, Occupational Therapists and Social Care Assessors.
This position is currently part time working 28 hours per week (4 days).
Team Goals
* The main aims of this team are to prevent, reduce and delay the person needing ongoing long-term support.
* To carry out timely assessments, reviews to ensure that needs are being met and short-term case management.
* The team will also be responsible for ensuring the person has the right level of care whether that’s following an incident that has led them to being admitted to hospital or following an episode of ill health or general decline at home.
* Assessment and Support Planning post hospital discharge or period of rehabilitation/recuperation at home/step down bed/flexi flat/residential
Expectations
* Anyone at assessment level will be expected to be a trusted assessor to be able to assess and provide small aids/adaptations.
* Multi-Disciplinary Teams (MDT) working will be key – this will include both professional MDT networking as well as working with Voluntary Community Services and social prescribers.
You will need to have sound knowledge and understanding of personal budgets; completing needs led assessments, Continuing Health Care and safeguarding process and procedures. You must be registered with Social Work England

Please note the salary quoted is per annum pro rata, as one of these positions is part time and the figures quoted are based on full time 36 hours per week.
