You will provide proactive, holistic case management whilst working with patients to help them set realistic goals. Utilising health coaching skills to actively engage with patient and their family to ensure that expectations are appropriately managed.You will complete occupational assessments and prescribe to patient equipment to, frail and elderly, palliative care and rehabilitation needs, to achieve quality and independence where possible.Due to the community working nature of the role, you will need hold a full UK licence and own a car with business insurance.
