InHome Therapy is seeking a compassionate, motivated, and licensed Home Health Physical Therapist (PT) to join our growing team. You'll provide one-on-one care to geriatric patients in their homes in and around Lincolnshire, helping them regain mobility, reduce pain, and improve their quality of life. Key Responsibilities:
* Deliver in-home physical therapy services
* Evaluate patient mobility, strength, and function to develop treatment goals
* Implement effective rehabilitation and therapeutic exercise programs for adult and geriatric patients
* Educate patients and families on recovery plans and preventive care
* Maintain accurate and timely home health care documentation

Requirements:
* Active PT license + BLS CPR
