Lynton integrated Community Nursing team is a small team in an isolated rural area covering a population of under 4,000 patients.
The role involves supporting the team to provide proactive, preventative and responsive nursing care to a wide range of patients.

Job responsibilities
You will provide nursing care for palliative and end‑of‑life patients, domiciliary and ambulatory wound care, catheter care, leg ulcer care, and minor injuries. As a Band 6 Community Nurse Specialist you will:
* Work clinically and provide day‑to‑day leadership to the team.
* Plan, implement and evaluate programmes of care using specialist assessment skills.
* Liaise with other professionals such as hospice nurses, social workers, GP, podiatry and tissue viability.
* Develop skills in structured clinical assessment for minor injury patients.
* Support a well‑leg/leg ulcer service and provide clinic services 3 days a week.
Working pattern: 37.5 hours per week.
