We are seeking a motivated person to join our ward teams in this exciting role as Care Coordinator, crucial to supporting the processes we have in place to deliver efficient and timely care.

You will be contributing to our commitment to providing the best possible patient experience to ensure good patient outcomes.

We are seeking a candidate with a can-do attitude, to join our teams trust-wide, within Wirral, Chester and East Cheshire based inpatient areas.

Main duties of the job

The Care Coordinator within an acute care ward will be responsible for coordinating aspects of patient care including liaison with other agencies both internal and external to CWP.

The post holder will be the central coordination point forward based multi-disciplinary activity, organising and supporting with ongoing regular meetings to facilitate progress to recovery and discharge with relevant CWP team members, relatives and carers and other external agencies.

Qualifications


Essential criteria

* Good general education
* Able to demonstrate a good command of English language, verbal and written.
* Knowledge of Office Administration/Custo mer service acquired through training or experience to NVQ level 3 or equivalent


Knowledge and Expertise


Essential criteria

* Knowledge of Health and Social Care administration and processes.
* Knowledge of Microsoft office applications, internet and email


Desirable criteria

* Knowledge/familiarity with medical terminology


Experience


Essential criteria

* Able to work well in a team.
* Experience of working in an Office Environment.
* Experience/knowledge of issues of working with confidential information
* Experience of working with healthcare professionals/previous experience in the NHS.


Desirable criteria

* Working in a busy and demanding environment.
