Overview
An exciting opportunity as a Band 5 Community Registered Nurse within the DN Treatment Clinics/Lower Limb Service (LLS) across Livewell Southwest Clinics. The service is a National Wound Care Strategy Programme (NWCSP) Implementation Site – Lower Limb. The post requires interest and knowledge in Tissue Viability and lower limb management to provide specialist care for lower limb health and skin care, working clinically under the guidance of Community Sisters & Team Managers. Effective communication, teamwork and health promotion/patient education are essential, with collaboration with other allied health professionals as Marjon University’s Sports Therapists support the Wellbeing programme.
Daily duties include care for non-housebound patients not available at GP surgeries, including wound dressings, pleural drain management, urinary catheter management, routine tracheostomy care, nephrostomy care and related therapies. The role involves working across Livewell footprint in various clinic settings, including informal settings within village halls. The role may require independent work during the day.
Responsibilities
* Provide holistic evidence-based / best practice care to patients in line with National and Trust policies/procedures and individual care plans.
* Specialist assessment, planning, implementation and evaluation of care packages with support from the Clinical Nurse Specialist/Team Manager/Team Leaders.
* Undertake comprehensive risk assessments and ensure patient safety; manage equipment and infection control with escalation as required.
* Plan and implement lower limb health maintenance, triage referrals, health promotion and discharge planning; maintain accurate, contemporaneous records.
* Act as a clinical advisor on health care issues within their area of expertise; act as a Link Nurse; provide education to patients, families and carers.
* Participate in audit, research, clinical governance and service development; contribute to policy development and efficient use of resources.
* Provide and receive education and continuing support to patients, families and carers; participate in clinical supervision/mentorship and regular appraisals.
* Ensure confidentiality and compliance with Data Protection, consent and information governance.
Qualifications and Experience
* Essential: Current UK Level 1 registration with the NMC; Full UK driving licence. Applications from newly qualified nurses are welcome.
* Desirable: Experience in community/clinic settings with complex nursing skills; community nursing experience; leg ulcer management qualification; mentorship qualification; ongoing development in tissue viability/lower limb management.
* Knowledge and Skills: Understanding of Government policies and changes in Primary Care; knowledge of local/national/NICE guidance for prevention and management of lower limb ulceration; evidence of recent study; excellent time management, interpersonal, delegation and IT skills; ability to travel across the Livewell footprint; multi-agency liaison skills; evidence of changing practice based on evidence.
Working Environment & Additional Information
Livewell Southwest staff are expected to work across a 7-day service. The post may include travel across Plymouth, Kingsbridge, Tavistock, Modbury and South Brent; driving is required. Note: the role includes exposure to physical and verbal aggression and handling of body fluids; appropriate safeguarding and infection control practices apply.

Salary: £31,049 to £37,796 per year (Agenda for Change Band 5, pro-rata). Contract: Permanent. Working pattern: Full-time, Part-time, Job share, Flexible working.
