Main duties of the job
In this role, you will take the lead in delivering high-quality asthma and COPD care, helping patients take control of their long-term conditions through personalised, proactive support. Working as part of a collaborative multidisciplinary team, you'll also play a key role in supporting and developing colleagues, while contributing to service improvement and innovation across the PCN. This is more than a clinical role; its an opportunity to shape care, drive improvements, and have a genuine impact on patient outcomes. You'll be part of a supportive, innovative team that is committed to delivering high-quality, patient-centred care while giving you the autonomy to develop your role and influence how respiratory services are delivered.

Job responsibilities
Key Responsibilities
* Deliver comprehensive asthma and COPD reviews
* Initiate investigations and perform/interpret spirometry (where qualified)
* Develop and manage personalised care plans, empowering patients to manage their conditions
* Identify and support patients at risk of respiratory disease
* Recommend and prescribe inhaled therapies in line with guidance (where qualified)
* Provide education on inhaler technique and medication use
* Deliver respiratory vaccinations, including influenza and pneumonia
* Undertake home visits where required
* Maintain high-quality clinical records
* Provide expert advice on long-term respiratory care
* Support QOF and enhanced service delivery
* Contribute to service development and continuous improvement
* Support and guide the wider nursing team

Qualifications
* Current NMC registration
* Degree in Nursing
* Diploma in respiratory management (Asthma and COPD)
