Pharmacy Technician Proactive Care and Frailty
The postholder will be a Pharmacy Technician and an integral part of the proactive care and frailty team.
This person will work autonomously to compile and manage their own caseload within a team.
They will provide support to patients with medications, conduct medication reviews, counsel patients on their medications, and, where possible, reduce medicines wastage.
Main duties of the job
They will be able to, or willing to train to, carry out basic patient observations such as:
* Heart Rate
* Blood Pressure
* Temperature
The successful candidate will make appropriate referrals into services or to other professionals involved with all aspects of that person's care. They will gain knowledge to aid with form filling and applying for relevant support for their patients.
The technician will answer queries from patients and other healthcare professionals regarding medications.
They must understand polypharmacy, deprescribing, and have the ability to refer appropriately to clinical pharmacists.
Attend the proactive care MDT weekly meetings.
This role involves independent travel to patients' homes for patient visits; a driving license and use of a car are essential.
The Pharmacy Technician will complete patient safety and medicines management audits and present the information for sharing with the team.
Accurate record keeping for all patient visits on the EMIS system is essential.
About us
Central Mendip PCN comprises 5 practices serving the population of central Mendip, including Mendip Country Practice (host practice), Coleford, Beckington Family Practice, Oakhill Surgery, Grove House Surgery, and Park Medical Practice, based in Shepton Mallet. Most staff are based at Highfield House on the Somerset County Council campus. The team includes clinical pharmacists, pharmacy technicians, care coordinators, health connectors, a proactive care nurse, and a nurse lead, supported by GPs from the practices. The team has been together since July 2019.
Job responsibilities
The Pharmacy Technician will undertake the following key responsibilities:
* Support effective medicines use through shared decision-making conversations with patients.
* Conduct medicine optimisation tasks, including reviews and reconciliation, utilizing consultation skills to work in partnership with patients.
* Support medication reviews and medicines reconciliation for new care home patients and synchronize medicines during patient transfers, liaising with community pharmacists.
* Provide expertise to address public health and social care needs, including lifestyle advice and tackling health inequalities.
* Maintain a central role in shared care protocols and liaise with specialist pharmacists for complex cases.
* Support antimicrobial stewardship initiatives to reduce inappropriate antibiotic prescribing.
* Assist in delivering medicines optimisation schemes and patient safety audits.

Qualifications
* Registered with the GPhC.
* Meets GPhC criteria for Pharmacy Technician registration.
* Enrolled in or qualified from an approved training pathway (e.g., PCPEP, MOCH).
* Working under appropriate clinical supervision.
* Experience in primary care.
* Knowledge of GP practice healthcare provision, QOF, and enhanced services.
* Understanding of NHS landscape, practice relationships, PCNs, and commissioners.
* Proficient with EMIS/SystmOne/Vision.
