The TAVI team consists of WTE TAVI CNS and 1 Senior Clinical Registrar and a few Honorary Consultants from Frimley Foundation Trust and Northwick Park. There is an administrator who supports the team in the patient’s journey. The team have responsibility for both in-patient and outpatient TAVI patients. New patients that are referred directly into the TAVI Service are triage and assessed for suitability to undergo TAVI procedure. The patients are given written and verbal information as what to do whilst they are on the TAVI waiting list and who and when to contact the TAVI team. Some patients are more closely monitored whilst on the waiting list.
The TAVI team work closely with other Cardiology teams, Cardiac Surgeons, Anaesthetists, ward staff, physiotherapist, occupation therapies, pharmacist bed managers, schedulers, 18 week pathway teams, GP’s and other external teams.

Main duties of the job
Duties and Responsibilities
·To co-ordinate incoming referrals, including collating previous investigations for patients referred to the TAVI team.
·To arrange assessment and outpatient appointments.
·To undertake nurse led clinics.
·To ensure that all appropriate information is correctly documented in the patient’s notes.
·To carry out the assessment of patients referred to the TAVI team in clinic and on the ward. This will include history taking, clinical assessment and review of previous investigations.
·To be involved in the clinical care of patients undergoing transcatheter valve procedures pre and post operatively.
·To present patients in the MDTs. This will include contacting referrers and patients with suggested outcomes/management plans.
·To organise patient follow-up and mandatory assessments or refer back to the team involved in the patient’s care
·To act as an expert resource for patients/relatives/colleagues relating to the care of their caseload of patients.
·To maintain patient’s confidentiality, privacy, dignity, religious and cultural needs at all times.
·To maintain an environment for safe and effective patient care.
·To assist in coordinating inter-hospital transfers for TAVI procedures

Operational Performance Management

·Management and Leadership
1. To develop and maintain clinical skills and knowledge in the speciality of Structural Heart Disease and cardiac care that is evidence based aiming to achieve a ‘best practice ‘principle.
2. To work with Structural Heart teams on the other sites, GSTT & Harefield.
3. To participate in the in-house adult education programme and to undertake informal teaching of ward nursing staff as required.
4. To act as a role model and clinical expert for staff providing education as appropriate.
5. To work in partnership with colleagues, having accountability for decisions made (in accordance with the NMC Code of Professional Conduct).
·To develop professional practise through reflection, participation on post registration courses and attendance at meetings and conferences.
·To identify and be responsible for own professional and managerial development in liaison with lead nurse cardiac nurse specialists .
·To contribute to the development of positive staff morale and team cohesiveness.
·To utilise appropriate resources within the Trust to provide a consultation, education, technical or information service as needed for self, staff, or patients.
Communication
6. To work effectively within the Trust's adult multidisciplinary cardiac team as required ensuring an individualised, evidence based optimum service for each patient and their family.
7. To act as advocate for cardiac patients enabling them to express their needs effectively when dealing with the multidisciplinary team.
8. To promote collaborative relationships with other health care professionals for the benefit of quality patient care.
9. To maintain effective channels of communication within the multidisciplinary team and promote effective interpersonal skills.
10. To liaise with allied healthcare professionals including GPs.
Quality, Risk and Governance
·Support the monitoring of compliance with internal and external governance and best practice requirements in the TAVI Service
·Support conflict resolution from patients, staff, suppliers, other internal and external service providers and partner organisations in the Service.
·Support in resolution of PALS issues.
·To ensure accuracy of documentation and to maintain an efficient and accurate record system with relevant information in agreement with the structural heart team.
Service development
11. To ensure the constant provision of effective, high quality nursing care to all cardiac patients.
12. To support and work with the TAVI multi-disciplinary team in the introduction of clinical changes.
13. To be actively involved in the review and development of cardiac related protocols and procedures in conjunction with other members of the multi disciplinary team and colleagues across the GSTT Sites.
14. To participate in the business planning process, helping to identify areas for service development.
15. Be innovative in widening professional boundaries in nursing/AHP.
16. Demonstrates advanced professional knowledge and skills for clinical competency and assumes responsibility for own professional development.
17. Undertake relevant training for electronic information systems in place & under development.
18. Following local guidelines order relevant tests and access results reported on electronic systems.

20. Review literature and leaflets for patients undergoing TAVI in conjunction with TAVI services across the GSTT Family
Research/Audit
21. Actively participates in clinical and nursing audit, quality assurance activities and research projects. Evaluates and implements findings with the multi-disciplinary team to maintain and improve care.
22. Identify areas where local audit is needed, conduct the audit and analyse the data with support from experienced team members.
23. Be involved in clinical audit evaluating factors for example patient satisfaction survey data.
24. To participate and be actively involved in multidisciplinary research studies of relevance to the care of patients.
25. To assist in introducing and applying research findings along with members of the team to enhance patient care.
