Main Duties
* Deliver expert healthcare services to patients.
* Provide clinical guidance to clinicians, allied health professionals, and patients.
* Provide comprehensive primary care within an integrated service comprising Primary Care, Substance Misuse, and Mental Health teams.
* Undertake physical health assessments, run clinics, and deliver treatment interventions.
* Demonstrate knowledge and understanding of health issues to guide staff on physical health and wellbeing matters.
* Ensure high standards of primary care services in a changing environment, keeping patient needs central.
* Maintain integration with the wider prison services and foster excellent communication with stakeholders.
* Promote high standards and professional culture within the nursing team.
* Participate in flexible working arrangements, including weekends and unsocial hours as required.
Detailed Job Description and Main Responsibilities
* Plan, deliver, and evaluate clinically appropriate interventions using evidence‑based guidance.
* Set up processes and protocols for assessment, planning, implementation, and evaluation of care.
* Safeguard vulnerable patients in line with CNWL Trust safeguarding policies.
* Apply local risk assessment and management procedures.
* Refer to other services for long‑term or complex care management.
* Coordinate multidisciplinary clinical practice to provide focused, integrated care.
* Use skilled interventions to de‑escalate aggressive patient behaviours.
* Collaborate with prison, education staff, and other disciplines for integrated care.
* Work within own competencies; communicate training needs to the team.
* Participate in local and trust audits; use research skills for service development.
* Liaise with external professional services, sharing information as appropriate.
* Work in a highly emotive environment with patients having histories of sexual offending or substance misuse; maintain professionalism.

Research, Audits, and Teaching

* Develop and facilitate training and support for all staff.
* Manage and provide supervision and training to nursing staff.

Person Specification
Qualifications
Essential:
* Registered Adult Nurse
* Evidence of personal and professional development in Nursing since registration
Desirable:
* Post‑registration qualification appropriate to speciality
* Mentorship or practice assessor certification
* Chronic disease management or long‑term condition qualification
Previous Experience
Essential:
* Practical experience of providing high‑quality care and exceeding expectations.
* Experience of providing and receiving supervision.
* Experience of running clinics.
* Substantial experience at Band 5.
* Ability to keep accurate records.
Desirable:
* Experience with IT systems.
* Experience working within a secure environment or prison.
Skills, Knowledge and Abilities
