PCN Social Prescribing Link Worker

Tadcaster and Selby Rural Primary Care Network (TaSR) is looking for a Social Prescriber to join our PCN team. TaSR are a semi‑rural group of 3 Practices (South Milford Surgery, Tadcaster Medical Centre and Sherburn Group Practice) with approximately 30,000 patients.

We are primarily looking for someone who will support and empower patients to access healthcare, community groups and statutory services for practical and emotional support. The role works closely with the PCN team, PCN Manager and member Practices. The role is also responsible for fostering strong long‑term connections within the community, 3rd party providers, local key personnel and council agencies.


Main duties of the job

Social prescribing is a non‑clinical role that empowers people to take control of their health and wellbeing by signposting and connecting them with appropriate community‑based services and activities. As a Social Prescriber, you will support individuals particularly those with long‑term conditions, mental health issues, social isolation, or complex social needs by helping them access support networks that improve their quality of life and reduce reliance on health and social care services.

This role requires the social prescriber to work efficiently within the fast paced and ever‑changing primary care environment, delivering high quality support in a responsive and effective manner.

The role is a hybrid approach to Social Prescribing with a mixture of face‑to‑face support as well as simple signposting to other services, ensuring patients voices get heard and they feel part of the process.

This role would suit someone who has been a Social Prescriber before or who has experience in the community sector.

Job responsibilities

Key Roles

* Support people by connecting and introducing them to community‑based activities, groups and services that meet their practical, social and emotional needs, including specialist advice services, arts and culture, physical activity, and nature and green based activities.
* Provide personalised support to individuals, their families and carers to take control of their wellbeing, live independently and improve their health outcomes. Help people identify the wider issues which impact on their health and wellbeing such as debt, poor housing, unemployment, loneliness and caring responsibilities.
* Manage and prioritise caseload, in accordance with needs, priorities and urgent support required by individuals.
* Refer people back to other health professionals and agencies where appropriate, making use of other local social prescribing services (Living Well).
* Work within local communities and develop/maintain good working relationships with the VCSE sector/local community groups and organisations.
* Enable patients to see the bigger picture one step at a time whilst improving patient outcomes.
* Work within appropriate safeguarding procedures and practices.
* Build effective relationships with key staff in GP practices, attending relevant meetings and becoming part of the wider network team.
* Work in partnership with other local agencies to raise awareness of social prescribing and to promote the service widely and its benefits.
* Use appropriate systems for data capture, case management, feedback and reporting, and adhere to data protection legislation and data sharing agreements.
* Work as part of the PCN Team as well as the wider General Practice team, sharing best practice and maintaining strong working relationships. Utilise other ARRS roles to signpost to.


Person Specification


Qualifications

* GCSE A to C in English and written communication
* Care and Support Management experience


Additional

* Experience working with vulnerable people.
* Good IT and Data Protection awareness
* Experience working with the public or patients.
* Experience working 1-2-1.


Experience

* General understanding of wellbeing and community based services in the local area.
* Experience working within a social care or community based role.
* Experience using clinical systems or complex patient record systems.
