Your application should reflect your genuine experiences and qualifications to ensure authenticity and credibility Job overview An exciting opportunity has arisen within The South Early Intervention in Psychosis Service (EIP) of Nottinghamshire Healthcare Foundation NHS Trust for a passionate and highly motivated Care Coordinator (Band 6) from either a Mental Health Nursing, Occupational Therapy or Social Work background to join our team The EIP service in Nottinghamshire is delivered through a multidisciplinary team of practitioners who provide comprehensive assessments and evidence-based treatments for people aged 14-65 who are experiencing a First Episode Psychosis (FEP) or an At Risk Mental State (ARMS) for the development of psychosis. The EIP team is a community-based service which operates Monday-Friday 9am-5pm. Advert We are looking for applicants with a keen interest in working with the client groups served by EIP and a desire to effectively support their families, to join a dedicated, enthusiastic and supportive team A good understanding of the principles, ethos, assessments and interventions associated with working within an Early Intervention in Psychosis team will be key. We provide additional training and support to contribute to the successful delivery of a high quality NICE concordant service, including Behavioural Family Therapy (BFT) and psychosocial interventions to our client group. Detailed job description and main responsibilities Day to Day Duties of the role are: Support and management of a caseload of service users who have been identified as experiencing their first episode of psychosis Assessment/screening of service users referred to the Early Intervention in Psychosis (EIP) Team To work as part of our integrated MDT, and to participate in clinical decision making process to determine the most appropriate management plans for all service users who are under the care of the team in collaboration with the service user and family/loved ones. Use of standard outcome measures to monitor recovery using the HoNOS, QPR and DIALOG Travel across Nottinghamshire to support the needs of service users, their family members and carers Use of digital technologies to support service need and delivery Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies. A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
