AHSCP – Community Nursing Services. Job Title: Band 5 Community Nurse. Location: Kincardine and Mearns District Nursing Team, covering Portlethen, Stonehaven, Inverbervie and Laurencekirk. Primary base to be confirmed. Hours: 22.2hours/week, working 0830-1700 Monday – Sunday.

In this role, you will deliver high-quality, person-centred care to patients within the community, helping them to manage long term conditions, recover from illness and live independently in their own home. As a community nurse you will work closely with a supportive multidisciplinary team, ensuring patients receive the best possible care in the comfort of their own environment.
Responsibilities
* Delivering a range of nursing interventions, including wound care, chronic disease management, medication administration and palliative care.
* Care Planning: Working alongside the DN/Senior Community Nurse to develop, implement and review care plans for patients.
* Multidisciplinary Collaboration: Work as part of a team with doctors, care management and other healthcare professionals to ensure holistic, patient focused care.
* Clinical Skills: Undertake clinical tasks such as injections, dressings and monitoring of vital signs.
We are looking for
* A registered nurse with a current NMC registration
* A genuine passion for providing care in the community setting and supporting patients to live independently.
* Strong clinical skills, particularly in areas such as wound care, medication administration and palliative care.
* Excellent communication skills and the ability to build relationships with patients, families and team members.
* Ability to work independently and manage your own caseload.
* Full driving licence and own car.
