In the role of Care Navigator you will work as part of a cross city team, providing a professional and effective response to our patients who access our services. This will be online or over the telephone and you will be responsible for signposting patients correctly to ensure they receive the right care, at the right time, with the most appropriate healthcare professional for their individual care needs. You will also provide key support to the acute care team for patients requiring access to same day care.
This is a fast-paced role where a keen eye for detail, communication skills and the ability to work as a team is essential.
The duties and responsibilities to be undertaken by members of the Access team may include any or all of the items in the following list. Duties may be varied from time to time under the direction of the Head of Access, dependent on current and evolving workload and staffing levels:
* You will deliver service excellence in a proactive and engaging manner following call pathway guidelines and protocols.
* Question patients effectively and document patient information with a high level of accuracy.
* Maintaining and monitoring the Practice appointments system.
* Proactively seek to develop and maintain knowledge and skill sets required within your role.
* Processing telephone requests for appointments, visits and telephone consultations and ensuring callers are directed to the appropriate healthcare professional.
* Computer data entry/data allocation and collation; processing and recording information in accordance with Practice procedures.
* Initiating contact with and responding to requests from patients, other team members and associated healthcare agencies and providers.
* Promote, advise and assist patients accessing services via our online portals.
* Work closely with the triage hub clinicians, offering administration support via in house systems.
* Utilise the enhanced/improving access services ensuring you are up to date with available signposting resources across cities.
* Providing clerical assistance to Practice and Trust staff as required from time to time, including word/data processing, filing, photocopying and scanning.
* Taking messages and passing on information.
* Provision of refreshments for staff and visitors as required; loading and emptying the dishwasher and keeping the kitchen area clean and tidy.
* Helping to maintain hygiene control measures.

Terms and Conditions
Salary:
£12.60 per hour, £24,966.07 gross per annum. Pay progression will be dependent upon annual performance appraisal.
Annual Holiday:
25 days plus bank holidays pro rata (holiday entitlement will increase as a reward for loyalty and service).
Training:
Induction training plus other annual training subject to an agreed personal development plan.
Working hours:
Full-time position – 38 flexible hours per week based around the opening times of Haxby Group.
The Surgeries will be open 8.00 am to 8.00 pm weekdays and until 1.00pm Saturdays. The post-holders exact weekly hours will be agreed according to service and individual needs and will include evening and weekend sessions (possibly on a rota). These hours may vary on a regular basis as dictated by service or individual requirements. To facilitate communication and training you will also be required to attend evening meetings, events and training that fall outside your normal working hours. These will be reimbursed as overtime paid at the normal hourly rate, or as time taken in lieu.
