Salary: Up to £59,000 per annum | Full-time hours
A forward-thinking Primary Care Network in New Malden is seeking a Clinical Pharmacist to join their team on a permanent, salaried basis. Key responsibilities
* Conduct patient-facing medication reviews and long-term condition clinics
* Support care homes, domiciliary visits, and post-hospital discharge medicines reconciliation
* Provide telephone support and advice for patients with medicines queries
* Contribute to safety audits, prescribing initiatives, and service development
1. What we’re looking for
* GPhC registered Pharmacist (Independent Prescriber desirable)
* Experience in primary care, hospital, or community pharmacy
* Strong clinical knowledge with a patient-centred approach
