What you will do:
* Undertake assessments and develop person-centred care plans in line with the Care Act 2014
* Facilitate safe hospital discharges, ensuring appropriate support is in place
* Work in partnership with NHS teams, families, carers, and external agencies to coordinate care
* Safeguard vulnerable adults, conducting risk assessments and interventions as needed
* Support individuals in regaining independence through reablement services
* Maintain accurate and timely case records in compliance with local policies
* Attend multi-disciplinary meetings to contribute to best outcomes for service users
