Overview
Bank Care Coordinator Band 6 (CPN/Social Worker) – based at Crombie House, Leighton Buzzard. The post is within the South Beds Leighton Buzzard CMHT and involves working with a multidisciplinary team including CPNs, Doctors, Psychologists, Support Workers and Occupational Therapists. You will be expected to apply Biopsychosocial intervention/recovery model within Community settings and support service users and carers in line with NHS Long Term Plan goals.
You will work in partnership with service users and carers, identifying and utilising strengths, and coordinating with other agencies and voluntary groups to promote independence. A good working knowledge of The Care Act, Mental Health Act, Mental Capacity Act/DOLS and other social care legislation is required.

Key Responsibilities
Quality of Service Provision
* To be responsible for an identified community caseload, ensuring high standard of professional conduct to comply with best practice and professional accountabilities.
* To deliver delegated statutory social care duties under the Care Act, Safeguarding and other relevant legislation in a complex, multi-disciplinary environment.
* To access appropriate care and support packages for people with mental health needs, and/or their carers, adhering to local authority procedures.
* To promote a service user focussed service.
* Participate in audits and surveys relevant to the team.
* To participate in development of new policies and procedures as required.
* Ensure that clients receive a culturally competent service and that Equal Opportunities and Human Rights are recognised and respected.
* To establish and maintain good relationships with service users, carers and voluntary sector colleagues.
* To provide best evidence practice to service users which complies with legislation, policies and procedures and relevant quality, governance and local and national guidelines.

Staff Management
* To provide supervision to all support staff as required.
* To mentor / supervise trainees as directed by deputy / team manager.
* To coordinate the day-to-day activity and manage the workload of the team in the absence of the deputy/team manager.
* Support newly qualified (ASYE) Social Workers following relevant training.
Financial Management
* To be responsible for the safe use of equipment and resources within the workplace.
Data Quality and Administration
* To adhere to the Trust's policies on Record Keeping and Care Programme Approach.
* Ensure data is input accurately into the relevant information system.
Person Specification
Education & Training
* Essential: Formal qualifications in health or social care, e.g., RMN, Dip SW, etc.
* Essential: Post registration experience.
Desirable
* Psychosocial Intervention Qualification and/or recognised psychotherapy training.
* Family intervention training.
Experience
Essential criteria
* Experience of working with people who have serious mental health difficulties in a multi-disciplinary/agency environment.
* Experience of working within a health trust or social services department.
* Experience of supervising junior staff and/or students.
* Experience of carrying out assessments to include physical health.
Desirable criteria
* Experience of working with people with first episode of psychosis and their carers.
* Working in a multi-cultural setting, including working with interpreters.
Knowledge and Skills
Essential criteria
* Knowledge or experience of early intervention in psychosis.
* Experience in preparing written and verbal reports.
* Creative and resourceful; ability to work in a team or on own initiative; excellent problem solving skills.
* Experience of promoting recovery; flexible and adaptable; ability to identify people’s needs and strengths.
* Demonstrate the provision of user centred care; ability to promote safety and positive risk taking; presentation and teaching skills.

Desirable criteria
* Experience of preparing and executing formal and informal presentations.
* Experience of participating in the setting up of a new service.
* Experience of working within a team caseload / assertive community engagement environment.
* Ability to facilitate development activities across organisations and to work in partnership, developing constructive relationships.
* Experience of liaising with statutory and voluntary sector organisations.
* Ability to work holistically with individuals and groups, addressing social, economic, and cultural factors affecting mental health.
