Main duties of the job
Welwyn Hatfield Integrated Community Team are seeking a motivated and enthusiastic Health Care Assistant to join our friendly, supportive and award winning Integrated Care team. You will be working alongside our trained nursing and therapy staff assisting them in the day to day care and management of our patients to ensure they receive safe, high quality nursing care.
As a Health Care Assistant, you will work alongside our diverse team comprised of nurses, paramedics, physiotherapists, occupational therapists and technical instructors. We work to provide evidence based nursing care to patients in their own homes. Our service operates 8am - 10pm 365 days a year. We offer flexible working patterns and are happy to consider full or part time applicants.

Job responsibilities
* To work within a team that provides an assessment and care service to patients with nursing / health needs.
* To visit patients in their own homes and provide appropriate care and support as defined in the Care Plan. This will include aspects of personal care, hygiene, uncomplicated nursing procedures, exercise, activity and treatment programmes prescribed by therapists. The aim is to rehabilitate and maximise the patients potential and where appropriate to support the carer.
* To support and enable patients in a range of activities to promote independent living and monitor their well-being. To undertake administration and clerical tasks that are associated with the care process including record keeping and use of computer.
Qualifications and Training
* NVQ Level III, or equivalent knowledge and experience
* Basic mathematical skills
Experience and Knowledge

* Awareness of personal care needs
* Awareness of rehabilitation skills

* Excellent written and verbal English skills
* Ability to work independently
* Ability to communicate when barriers to understanding exist
