More detail about the role This role is based within the Community Rehab team which includes Physiotherapists, Occupational Therapists, a Rehabilitation Nurse and Assistant Practitioner. The post holder should be highly motivated to provide patient-centred care in patients homes, nursing and residential homes. The service provides support and rehabilitation to patients with long term and acute conditions, falls prevention and care of the older person, to achieve their full potential. The post holder will ideally have experience in a health and social care setting, supporting patients independence. There is day to day clinical support from Clinical Leads as well as a structured in-service training programme to help support and guide your development. We link closely with our wider colleagues in the Community Nursing teams as well as social care. We also support the Community Urgent Response team at times of need and the successful candidate is also expected to contribute to the 7-day service between the hours of 08:00 - 20:00 on a rota basis.
