Responsibilities
* Deliver assessment, commissioning support and strength‑based support to young people entering adulthood, including those with learning disabilities, physical disabilities, mental health conditions and ASD.
* Work within the legal framework of the Care Act, Mental Capacity Act and Mental Health Act.
* Safeguard adults and collaborate with multi‑agency partners, including Children’s Social Care, education and health services.
* Support young people in achieving their aspirations and outcomes at this pivotal life stage.
* Contribute to service development, policy and practice improvement.
* Receive regular supervision and access to training and professional development opportunities.
Qualifications & Requirements
* Social Work Qualification and registration with Social Work England.
* Minimum 2–5 years experience as a Social Worker, preferably in adult social work.
* Knowledge and experience of safeguarding adults and working within multi‑agency arrangements.
* Strong communication, time‑management and IT skills.
* Full driving licence with access to transport (disabled drivers supported with taxis).
* Availability to work 37 hours per week, full time, in a hybrid arrangement with occasional travel to Meadow House.
