As an Advanced Clinical Practitioner on the wards at Sittingbourne Community Hospital, you'll join a team of support workers/HCAs, physiotherapists, occupational therapists, and nurses who are focused on exceeding the needs of patients within our North Kent community.
This role covers services in Swale, and you may need to work at both Sittingbourne Community Hospital and Sheppey Community Hospital.
You will support the delivery of care to a range of complex patients, including those with life-limiting conditions such as diabetes, in-dwelling long-term catheters, urgent wound care, etc.
This is a weekday service, working Monday to Friday, 9am-5pm. No weekends or bank holidays required.
Main duties of the job
As an Advanced Clinical Practitioner, you will work closely with a multidisciplinary team, practicing autonomously and flexibly. Practitioners should have significant experience in primary care or A&E/minor injuries units. You will work within the nursing team with the service manager and clinical lead to deliver robust clinical care.
