To provide evidence-based, high-quality nursing care to patients in a GP surgery setting, supporting the delivery of primary care services including chronic disease management, health promotion, and treatment room procedures. The post holder will work collaboratively with the multi-disciplinary team to meet the needs of the practice population.
We welcome applications from experienced practice nurses and also from qualified registered nurses with an interest in developing their career in primary care nursing. Tailored training and mentorship will be provided to suitable candidates without prior practice nurse experience.

Job responsibilities
Key Responsibilities -
Clinical Duties -
Manage and deliver chronic disease management clinics (e.g. asthma, COPD, diabetes, hypertension)
Provide wound care, including dressings, suture removal, and leg ulcer management
Undertake cervical cytology testing
Administer immunisations and vaccinations (childhood, travel, flu, etc.)
Conduct Health Checks and offer lifestyle and health promotion advice
Support family planning
Assist with minor ops and plastics surgery sessions
Perform ECGs, venepuncture, urinalysis, and blood pressure monitoring
Conduct health checks and reviews for at-risk populations
INR monitoring
Work as part of the nursing team to plan and deliver services
Liaise with GPs, HCAs, Nurse Practitioners, Paramedic Practitioner, pharmacists, and community services
Participate in multidisciplinary team meetings and contribute to case management
Professional Development -
Maintain NMC registration and meet revalidation requirements
Undertake mandatory and practice-specific training
Engage in clinical supervision, audits, and quality improvement initiatives
Infection Control -
Support patients and colleagues in adopting sound infection control measures.
Apply infection control measures within the practice according to local and national guidelines.
Assist the infection control lead in undertaking audit and ensuring compliance with required standards
Quality Improvement and compliance -
Undertake audit to identify quality improvement opportunities
Work with the team to deliver quality improvement
Work with the practice to ensure evidencing and achievement of regulatory compliance
Person Specification
Training and Support
* For nurses new to primary care, the practice will provide -
* A structured induction programme
* A named mentor or supervisor
* Access to external courses - e.g. cervical screening, immunisations, chronic disease management
* Protected learning time
Experience
* Experience in Nursing (e.g., hospital, community or practice)
* Excellent communication and interpersonal skills
* Ability to work independently and as part of a team
* Good organisational skills and ability to prioritise workload
* Commitment to patient-centred care and confidentiality
* Previous experience in a GP surgery or primary care setting
* Experience managing chronic diseases independently
* Cervical cytology trained and immunisation trained
* Knowledge of EMIS and primary care IT systems
* Mentoring or preceptorship experience
* Evidence of continuing professional development
* Participation in audit or service improvement initiatives
Qualifications
* Registered Nurse with current NMC registration
* Qualifications in chronic disease management, contraception or minor illness
Other Requirements
* Willingness to undergo further training specific to practice nursing
