About The Role
This is a permanent full‑time Social Worker post within Derbyshire County Council’s Leaving Care Service. The role is based at the Chesterfield care leavers hub (POD), providing corporate parenting and trauma‑informed practice to young people aged 16–18.

What you will be doing
* Provide a quality social work service that supports children to achieve their potential and positive transitions to adulthood.
* Effectively manage a portfolio of young people to achieve the best outcomes.
* Model best practice, mentor others and maintain a commitment to corporate parenting.
* Work in partnership with children and families, building relationships, identifying needs, and developing responsive plans.
Skills and experience
* BA Honours in Social Work and current Social Work England registration.
* Excellent communication and interpersonal skills with a person‑centred approach to building rapport.
* Experience of relationship‑focused social work and reflective practice.
