The successful candidate will have excellent interpersonal and communication skills, and be organised, patient and empathetic. They will have experience of working in health, social care or other support roles including direct contact with people, families, or carers

JOB DESCRIPTION
Job Scope
The postholder is a pharmacist, who acts within their professional boundaries,supporting and working alongside a team of pharmacists in general practice. Inthis role they will be supported by a senior clinical pharmacist who willdevelop, manage, and mentor them.
The post holderwill work as part of a multi-disciplinary team in a patient-facing role. Thepost holder will take responsibility for areas of chronic disease managementwithin the practice and undertake clinical medication reviews to proactively manage patients with complexpolypharmacy.
The postholder will provide primary support to general practice staff with regards toprescription and medication queries. They will help support the repeatprescription system, deal with acute prescription requests, and medicinesreconciliation on transfer of care and systems for safer prescribing, providingexpertise in clinical medicines advice while addressing both public and socialcare needs of patient in the GP practice (s).
The postholder will provide clinical leadership on medicines optimisation and qualityimprovement and manage some aspects of the quality and outcomes framework,investment and impact fund and enhanced services.
The postholder will ensure that the practice integrates with community and hospitalpharmacy to help utilise skill mix, improve patient outcomes, ensure betteraccess to healthcare, and help manage workload. The role is pivotal to improvingthe quality of care and operational efficiencies, so requires motivation andpassion to deliver an excellent service within general practice. The postholder will be supported to develop their role to become a nonmedical prescriber.

Qualifications
* Mandatory registration with the General Pharmaceutical Council Completion of an undergraduate degree in pharmacy and
* registration with the General Pharmaceutical Council.
* Demonstrates and understanding of, and conforms to, relevant
* standards of practice.
* Follows professional and organisational policies/procedures
* relating to performance management
* Membership Primary Care Pharmacy Association (PCPA)
* Membership of the Royal Pharmaceutical Society
* May hold or be working towards an independent prescribing
* qualification
* Clinical Diploma
Experience
* Minimum of two years experience as a pharmacist, demonstrated
* within a practice portfolio
* Experience and an awareness of the breadth of common acute
* and long-terms conditions that are likely to be seen in general
* practice
* Demonstrates ability to integrate general practice with community
* and hospital pharmacy teams community groups
* Experience of partnership/collaborative working and of building
* relationships across a variety of organisations
