Job Overview
This is a fixed term/secondment vacancy for 12 months until 31/10/2026 due to the needs of the service.
If you are a Hywel Dda University Health Board employee and are applying for this post as a secondment, agreement for the secondment is needed from your current manager prior to application.
We are looking to recruit a Health Care Support Worker to join our Crisis and Home Treatment Resolution team. We are a well-established community mental health team.

The community mental health team work with people who are experiencing moderate to severe mental health difficulties. Supporting people through their journey of recovery from mental health difficulties.

The role of the support worker is to be part of/compliment the community mental health multi disciplinary team in delivering interventions and support to promote the recovery people experiencing mental health difficulties.

The role of the support worker within the CRHT/ community mental health team is varied and there are opportunities for staff to develop skills in supporting therapeutic interventions, working under the supervision of a team of experienced senior mental health practitioners. The team includes a number of very experienced support workers who the successful applicant will work alongside.

Main duties of the job
Supporting senior mental health practitioners to deliver responsive and effective care during assessment and treatment to a designated caseload of individuals experiencing moderate to severe mental health difficulties.

Working in a compassionate and non-judgemental manner with individuals in their own homes, in the community and in health settings.

Accurately recording contacts with individuals in clinical notes in a timely manner. Support the senior practitioner to complete all necessary administrative tasks.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.

* Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest
* Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire
* Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings
* 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers
* Numerous mental health and learning disabilities services
