We are recruiting Support Workers to join our Physical & Complex Care Supported Living Service. You will support adults with complex physical needs, Palliative end of life care and Dementia, delivering high-quality, person-centred care that promotes dignity, choice, and independence. You will work within a skilled, supportive team providing compassionate, structured support every day, including:
* Intimate personal care
* Moving & assisting (safe equipment use)
* Palliative and End of life care
* Medication administration
* Supporting meaningful, independent living
Key Responsibilities
* Deliver respectful, person-centred care
* Support Palliative end of life care and dementia-related needs
* Carry out safe moving & assisting
* Assist with appointments, meals, domestic tasks, and money management
* Promote wellbeing, independence, and positive routines
* Follow care plans, policies, and health & safety requirements
* Maintain confidentiality and professionalism
* Work collaboratively as part of a supportive team
