Are you soon to qualify as a Dietitian or are you a Band 5 looking to gain more experience? We are seeking a motivated, enthusiastic individual with excellent communication and team working skills. This position is part of an expanding team with opportunities to help shape future service provision.
You will work alongside Specialist Dietitians with expertise in various specialties, providing care for vascular, urology, and elderly patients. You will also gain experience in supporting food policy initiatives, promoting MUST screening, and service development.
Responsibilities include:
1. Managing a designated caseload of patients with diverse and complex dietetic needs.
2. Developing individualized holistic care plans addressing nutritional status, weight, dietary modifications, lifestyle factors, behavior change, education, physical activity, and condition management where appropriate.
3. Supporting patient care through implementation of dietetic care pathways.
4. Acting as a nutrition expert for medical staff, nursing staff, and other healthcare professionals.
5. Liaising with healthcare professionals within and outside the Trust.
