Provide a highly specialised clinical service to a designated ward on a rotational basis in accordance with professional, departmental and Trust policies. This will include:- - Undertaking medicines reconciliation - Reviewing medication to ensure safe and effective use. - The formulation of individual pharmaceutical care plans. - Reviewing and updating care plans depending on response, results of investigations and adverse effects. - Therapeutic drug monitoring for specific drugs, when requested. - Educating patients about their drug therapy. - Developing evidence-based treatment protocols for use either within the directorate or across the Trust where appropriate. - Facilitating implementation of local and national guidelines where appropriate. · Ensure efficient use of resources by assessing patients’ own drugs for suitability for re-use during the inpatient episode. · Assist in promoting safe prescribing and administration of drugs through clinical screening and monitoring of prescriptions and prescription charts, taking appropriate corrective action where necessary and making records of such activities. · Provide specialist and complex pharmaceutical information to medical and nursing staff and patients within designated wards and to General Practitioners and other staff within the Trust where appropriate. · Support nursing staff and Medicines Management pharmacy technicians by providing clinical pharmacy advice to the patients’ self-administration initiative and to undertake ongoing evaluations of individual outcomes of patient self-administration. · Provide specialist pharmaceutical information and advice to patients, prescribers and nursing staff and to promote safe, effective and economic use of drugs. · Help identify clinical risks relating to the use of medicines, in pharmacy and elsewhere, and propose risk reduction measures
