About The Role
Are you passionate about supporting people with care and support needs living in North Northamptonshire to progress and live independently? As part of our transformation, we are focused on assessments and support planning that identify an individual’s strengths and the outcomes they want to achieve. The Assessment and Enablement role in the team will undertake detailed assessments to enable people to move accommodation, make changes to their lives, or ensure providers are meeting identified outcomes set by, and for, the individual. These assessments are in line with statutory requirements and best practice.
