Awaken Mental Health is a mental health organization dedicated to supporting individuals facing mood disorders, anxiety, attention‑related issues, and co‑occurring conditions such as substance use disorders.
Awaken is a division of TASC, a long‑standing drug and alcohol agency that has been providing mental health case management in Berks County since 2000.
Job Summary
This role reports to the Awaken Program Supervisor and/or Awaken Program Director. It is an hourly / non‑exempt, full‑time, mobile position with flexible scheduling (e.g., 8 a‑4 p, 9 a‑5 p, or 10 a‑6 p). The work schedule is 37.5 hours per week, starting at $22.00/hr with potential increases after 90 days to $23.00/hr and eventually $24.00/hr. The employee may receive bonus pay for additional billable hours with prior approval. Mileage expenses will be reimbursed at the IRS rate.
Caseload can be 30 cases or fewer. Candidates are expected to have 6 months to 1+ year of experience in mental health and possess a BS degree.
ESSENTIAL DUTIES & RESPONSIBILITIES
Provides case management services for up to 30 individuals seeking assistance, stabilization, and management of a diagnosed mental health disorder.
Meet with clients in their homes, community, or other appropriate locations; link clients to supportive services; may transport clients as needed.
Develop Individualized Service Plans (ISP) within 30 days of case assignment and update the ISP at least every 90 days.
Document contact with each client on a bi‑weekly (ICM) or monthly (RC) basis in accordance with Awaken contact standards; update client service level every six months.
Work collaboratively with service providers to assist clients with problem solving and resource acquisition to reduce hospitalizations, improve living skills, and promote stable and healthy living.
Utilize Motivational Interviewing techniques and discuss treatment options with each individual, providing support, encouragement, and hope.
Use worksheets, including strength‑based assessments, to identify strengths, weaknesses, supports, and obstacles to attaining goals.
Enter timely and thorough documentation into CPR‑Web within 24 hours of all client interactions.
Make assertive documented attempts to engage new client referrals within 48 hours of assignment.
Respond promptly (within 24 hours) to client emergencies such as hospitalizations, involuntary commitments, medication refills, and family crises in accordance with CCBH and OMHSAS standards.
Orient new clients to services, procedures, and requirements to maintain case management services.
Submit required paperwork timely as directed by supervisor or Program Director.
Participate in regular supervision sessions; keep supervisor informed of client status and notify of problems or crisis situations.
Maintain strict confidentiality and ethical standards during interactions with clients, family, attorneys, treatment providers, case workers, and referral sources.
Attend all meetings, conferences, and trainings related to job functions.
Maintain and develop mental health and substance use professional knowledge through required trainings, workshops, seminars, conferences.
Represent TASC positively and professionally at all times.
Perform other duties as assigned by supervisor or Program Director.
ADDITIONAL REQUIREMENTS & JOB EXPECTATIONS
A. Productivity
Effective 1.01.2025, case managers must maintain a monthly billable productivity level of at least 55%.
New hires are expected to attain this level by the end of their probationary period.
Case managers with low productivity may be placed on a Performance Improvement Plan.
Three months of low productivity within a six‑month period may lead to disciplinary action up to termination.
Increasing productivity immediately after a low month may negate the low month if the average equals 55% over two months.
Low productivity may be excused by the Awaken Program Director and TASC Executive Director due to exigent circumstances.
B. Additional Expectations
a. Daily Work Schedules
Work 37.5 hours per week. Schedules are flexible and may require evening or weekend hours.
b. Client Cancellations
Make a concerted effort to fill vacant time and document contact attempts. Minimum monthly billable productivity remains required.
c. Daily Correspondence
Respond promptly within the same workday to emails, telephone, and text messages from supervisor or Program Director.
d. New Client Referrals
New clients are assigned by Program Director. Refusal of new assignments without approved exigent circumstance may result in disciplinary action up to termination.
JOB QUALIFICATIONS

REQUIRED EDUCATION & EXPERIENCE
Bachelor’s degree in social work, psychology, or related field.
1 to 3 years of experience in mental health or with clients with mental health diagnoses.
Demonstrated responsiveness, depth of knowledge of mental health disorders, and thorough crisis handling.
Working knowledge of mental health advocacy and treatment services in Berks County.
Strong interpersonal, communication, organizational, and self‑motivation skills.
Adherence to all applicable federal and state laws and regulations including confidentiality, anti‑fraud, HIPAA, and OMHSAS standards.
Proficient computer skills and solid knowledge of Microsoft Office (Outlook, Excel, Word).
ESSENTIAL PHYSICAL REQUIREMENTS
Regular speaking and hearing required. Frequent standing, walking, sitting, hand use, and reaching. Occasionally lift up to 10 pounds, infrequently up to 25 pounds. Vision: close, distance, peripheral.
OTHER REQUIREMENTS
Valid criminal background and child abuse clearances, post‑offer drug screen, valid driver’s license and insured motor vehicle required.
Bonus Pay
* Level 1: 350–374 billable units = $5 per unit.
* Level 2: 375 billable units and above = $10 per unit (payable to 350).
* All casework and documentation must be completed in full for bonus pay.
