Job Overview This is a very exciting opportunity for an experienced Physiotherapist to support the development, implementation, continuous monitoring and improvement of the Active Together Programme for a 12 month fixed term contract to cover maternity leave. The role of Specialist Physiotherapist is to help deliver and sustain the success of the exercise element of the Active Together Programme at Airedale, working collaboratively with the Clinical Specialist Physiotherapist (Band 7) and Operational Manager. The Active Together programme runs alongside the existing diagnostic and treatment pathways to ensure patients are optimally prepared for treatment and beyond. It aims to maximise resilience to treatment, improve longer term health, minimise side effects and empower patients to be partners in their healthcare. The programme combines physical activity, nutritional support and psychological support alongside the clinical pathway. This is a funded 5-year staged implementation programme. Patients receive individual assessments and are triaged into a support pathway suitable to their needs. This is delivered within locality or selected facilities closest to the patient's home or may be a self-management pathway. You will be joining a small friendly team based at Skipton Hospital consisting of the Operational Manager, another Physiotherapist, a Dietitian, a Psychologist, and plans to recruit a Project Co-Ordinator, and Fitness Instructors in the future. Main duties of the job Support the implementation of the Active Together prehabilitation and rehabilitation services at Airedale NHS foundation trust. To frequently undertake the comprehensive assessment of patients, with a complex presentation and multiple comorbidities. To analyse the assessment to formulate individualised, evidence-based treatment plan, using own clinical reasoning and a range of treatment skills and options to formulate a specialised programme of care. Prioritising this efficiently considering clinical need, priorities and time management. Be an integral part of the Cancer Prehabilitation team working within the MDT to deliver a high-quality effective service, including identification of dietetic and psychological needs of these patients, liaising with the appropriate teams when required. Detailed Job Description And Main Responsibilities If you are applying internally please make ensure you have your line manager's support for this secondment. Provide expert advice on the most up to date rehabilitation treatment and management for patients referred into the service. Assess and treat own caseload of patients/clients in various settings including one to one appointments, and group sessions. To receive and communicate complex, sensitive or contentious information to patients and carers. To have excellent verbal and non-verbal communication skills of motivation, explanation and reassurance to agree and gain co-operation of patients and carers in their treatment programmes Contribute to the reduction of recurrence of cancer and increased survival rates in West Yorkshire through the design, delivery, and translation of physical activity interventions across the cancer rehabilitation pathway. To organise and deliver group physiotherapy activities, considering varied patients needs, including both physical and mental needs. Adapting sessions depending on patients changing needs. Providing training, education and promotion of the Active Together Programme to the wider MDT and working alongside the wider MDT to provide excellent, personalised patient care. Please Refer To The Job Description For More Information. Person specification Qualifications Essential criteria Degree or equivalent pre-registration award in Physiotherapy. State registrations with the HCPC. Desirable criteria Completion of course in Professional Practice education. Recent post graduate courses relevant to the Cancer/Cancer rehabilitation. Experience Essential criteria Relevant and recent experience working including wide clinical experience at Band 5 level Experience of multidisciplinary team working Experience of Oncology and Palliative care work. Desirable criteria Experience working at band 6 level Experience of working on Projects and/or service improvements Knowledge Essential Criteria Knowledge and clinical reasoning skills to support clinical practice in cancer care and rehabilitation. Knowledge of treatments and consequences of treatment related side effects on a person living with and beyond cancers. Knowledge of the roles of other professionals and the principles of partnership working. Desirable criteria Knowledge of relevant national and local legislation relevant to cancer care and rehabilitation. Specialist knowledge, application, and analysis of standardised and non-standardised assessment tools. Skills Essential criteria Skills to work as of a part team. Excellent planning and organisational skills Excellent communication and counselling skills including observation, listening and empathy. Desirable criteria Ability to chair meetings and facilitate workshops effectively. Personal attributes Essential Criteria High degree of self-motivation and initiative. Supportive, approachable, tactful, and diplomatic. Highly flexible to meet own and service needs and be open to new ideas. Works independently within parameters set by codes of conduct, professional guidelines, and policies. Other Essential criteria Current UK driving license Access to own transport
