Job Overview – Cardiac Disease Risk Management Nurse
Band 6 – Part Time
Location: Wexham Park Hospital
An exciting opportunity has arisen to join and support the expansion of our Cardiac Disease Risk Management Service at Wexham Park Hospital.

We are looking for an enthusiastic and motivated nurse with a strong background in cardiology, excellent communication, and effective time management skills. This role offers the chance to make a real difference to patients and their families by supporting their recovery and long-term health following a cardiac event.

About The Role
Deliver high-quality, evidence-based care to patients referred to the service.

Provide education, advice, and support to patients and their families.

Contribute to the ongoing development and improvement of the service.

Work flexibly, with occasional evening and weekend commitments.

About You
Registered nurse with experience in cardiology.

Knowledge of both national and local cardiac care priorities.

Previous cardiac rehabilitation experience is desirable.

Strong interpersonal skills, with the ability to engage and motivate patients.

Main duties of the job

To participate in the delivery of the Cardiac Disease Risk Management Service across the acute and community setting, teaching, advising and supporting patients and their families following a cardiac event

To take an active role in the development of the Cardiac Disease Risk Management Service

To liaise with other health care professional to ensure a seamless patient pathway

Qualifications
Essential criteria

* RGN Level 1
* Current NMC registration
* Evidence of continuing professional development

Desirable criteria

* Degree level study
* Confident in teaching

Skills and Knowledge

Essential criteria

* Assess, plan, evaluate & implement care ensuring current research used
* Time management and delegation skills
* Proficient presentation skills
* Effective working with multidisciplinary teams
* Ability to adapt to change within the working environment

Experience
Essential criteria

* Experience in mentoring and or supervision of students and new starters
* Recent post registration experience in acute cardiology
* Teaching and supervising of junior staff

Desirable criteria

* Previous Cardiac Rehabilitation experience
* Experience of audit/research
