Join to apply for the Community Psychiatric Nurse role at Nottinghamshire Healthcare NHS Foundation Trust.
This is an exciting opportunity for experienced Mental Health Nurses to join our County South LMHT teams, with vacancies within the Broxtowe & Hucknall LMHT and Gedling LMHT. You will work in a community setting with adults across the Nottingham area, within SMI pathways (mood affective disorders, complex psychosis, community rehabilitation and non-psychosis). You will have access to training to develop skills in working within a specific pathway and support from a very collaborative, multi-disciplinary team.
Responsibilities
* Hold a caseload and work with patients and their families to support personalised care plans and provide specialist interventions.
* Promote recovery and, through partnership with other agencies and primary care, support timely and safe discharge from secondary care.
* Complete triage assessments and more detailed initial assessments of new referrals.
* Provide support via the team’s duty system, either from telephone support or face to face.
* In this leadership role, develop specialist skills in working with patients in at least one pathway within the team.
* Support students and more junior staff within the team through clinical supervision and mentorship; may be asked to line manage some staff.
* Work within a dynamic multi-disciplinary health team with excellent communication and interpersonal skills; manage complex cases in partnership with other agencies.
Qualifications and requirements
* Experienced Mental Health Nurse with ability to work within a community setting.
* Willingness to undertake training to develop pathway-specific skills.
* DBS check required; costs are deducted from salary following appointment. DBS application costs are £26.40 (standard) or £54.40 (enhanced). You may enrol in the DBS Update Service; an annual fee of £16 applies.
* Ability to work with adults across pathways and to provide person-centred care in collaboration with partners and primary care.
* Strong communication and interpersonal skills; ability to support students and junior staff through supervision and mentorship.
