Job Summary
This is an exciting opportunity to join the Waterloo Practice, a forward-thinking primary care practice in Huddersfield. The role is a Practice Nurse position, part-time, with a closing date of 31 May 2026. You will work within a multidisciplinary team of GPs, ACPs, pharmacists and other nurses to manage patients with long-term conditions. Salary will be negotiated based on experience.
About the Practice
The Waterloo Practice serves a population of up to 9,700 patients, consistently achieving high QOF figures and a good CQC rating. The practice uses TPP SystmOne and offers a wide range of services including chronic disease clinics, antenatal and postnatal care, minor surgery, vaccinations and sexual health services.
Job Responsibilities
* Manage patients with long-term conditions such as diabetes, respiratory and cardiovascular diseases.
* Provide health promotion and screening, including travel health, cervical cytology, vaccinations for baby, childhood and adults.
* Actively participate in the Primary Care Network.
* Support and mentor colleagues within the practice.
* Use TPP SystmOne system for clinical records and reporting.
* Commit to personal professional development.
Qualifications
* Qualified nurse and registered with the NMC.
* Experience in general practice nursing.
* Experience with cervical screening and complex wound care.
* Knowledge of long-term condition management – diabetes, respiratory, cardiac and vascular conditions.
* Experience with immunisations and vaccinations.
* UK professional registration.
* Completed Disclosure and Barring Service check (Rehabilitation of Offenders Act (Exceptions Order) 1975).
